If you require any vaccinations relating to foreign travel you need complete a Travel Vaccination Risk Assessment Form and then make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

Vaccinations should be given approximately 6 weeks before you travel, however can be given up to 2 weeks prior to travel if necessary.
Please complete and submit the Travel Vaccination Risk Assessment Form below as fully as possible to the best of your knowledge, ideally at least 8 weeks prior to travel. Please ensure you include a daytime telephone number as the nurse may need to contact you.
When completed bring to surgery and make an appointment to see the Practice Nurse giving the nurse enough time to research what vaccinations are required.
If you require malaria tablets due to your destination this will be a private prescription costing £20.00. There will then be additional costs when purchasing the medication from your chosen pharmacy. There are also a limited number of vaccinations not available on the NHS for which there would be a charge, you would be informed of this and payment due prior to treatment commencing
We also offer Yellow Fever vaccination with a fee of £65.00. This is also available to patients who are not registered with us. Please contact surgery if you wish to make an appointment for this.
If we are unable to provide an appointment, or vaccinations are needed less than 2 weeks prior to travel, then it may be necessary to contact MASTA.
