New Patient Registrations
The Practice welcome new patients who live within our practice area. As a guide this covers the areas of Ossett, Horbury, Horbury Bridge and parts of Lupset (west of Broadway), Shaw Cross (south) and Chickenley (east of Chickenley Lane). Click on the link below to see an illustration of the practice area, the area marked in black is the main practice area. The area marked in red is the extended practice area; patients currently registered at the practice that move into this area can remain registered.

Patients wishing to register with the practice will need to complete a registration form and New Patient Pack that can be printed off via the links above or are available from reception.
If you take any regular medication please bring details of these with you.
Following completion of the form all new patients over the age of five may be invited to book an appointment for a health screening examination.
Medical treatment is available from the date of registration. Please contact reception for further information.
Upon registering with the practice patients will be allocated a named accountable "usual" GP. Please ask reception if you either wish to check who this is or if you would like it to be changed.
